Implementation notes
Point values are from Arozullah et al., Ann Intern Med 2001, Table 4 (preoperative transfusion of more than 4 units = 3 points), and the class rates from its abstract; the same point table appears in Moraes et al. 2022 (PMC10578803, Table 3). Licker 2007 (PMC2699974) gives 4 points, but its table has other transcription errors, so it was not followed.
