Implementation notes
Risk-group cut-offs (very low ≤ 1.5; low > 1.5–3; intermediate > 3–4.5; high > 4.5–6; very high > 6) are from Greenberg et al., Blood 2012, Table 4, and median survivals from Table 5, so a score of exactly 1.5 is very low.
