Implementation notes

Albumin/bilirubin/INR < 1.0 set to 1.0; creatinine set to 1.3 if > 1.3 or after ≥ 2 dialysis treatments or 24 h CVVHD in the prior 7 days. The policy uses the minimum CDC height or weight z-score in place of the old yes/no growth-failure flag; enter it from a CDC 2000 LMS calculation. For creatinine below 0.2 mg/dL the term 1.453 × ln(0.2) is used, as printed in the Board-approved OPTN policy notice (June 2022); the current policy text prints ln(.02), which would make the score jump at 0.2, and is treated as a typographical error. What the OPTN allocation system itself computes below 0.2 is unresolved. No published worked example was found; tests are hand-computed.
